If you have received approval of your change of deadline date with a d'Anelli Bridal consultant, please read the following agreement below and submit the form.
I, (Bride's Name) have decided to extend the deadline date for my Bridesmaids / Jr Bridesmaids / Flower girls to purchase their dresses. I acknowledge that the shipping dates provided are from the manufacturer and d'Anelli Bridals will not be held responsible for their production schedule. I fully accept that additional charges (RUSH and/or shipping upgrade from the manufacturer) may occur as a result of making this change and agree to pay or have my bridesmaids pay the charge(s).
